Document how to add a maintainer/project

Thanks to !5605 (merged), we have a central place to list projects and maintainers. @grzesiek, would you please update the README.md to document how to add a project and/or maintainer? I assume it's as simple as updating data/team.yml and data/project.yml. Thanks.
