Remove Vulnerability Management Survey

Why are we doing this work

The Vulnerability Management Survey has been live since March of 2021 and we should consider removing it. It is also controlled via the vulnerability_management_survey feature flag

Remove all frontend and backend code associated with the survey banner, including th e vulnerability_management_survey feature flag.
