Periodically re-establish database connections from app tier

To implement auto-rebalancing of pgbouncer client conns as described in gitlab-com/gl-infra/production#1390 (closed).

I'm not sure if this is possible, or how best to implement it (perhaps a configurable TTL for connections)?
