Report only new inconsistencies

When a new schema inconsistency is found, we create a new GitLab Issue with the schema inconsistency description.
Example: #408040 (closed)

If the schema inconsistency is already tracked in the system, we only create a new issue if the issue is not opened.
Example: If the GitLab issue is closed, we will generate a new GitLab issue.
