Add title and icon to directory/file pages

Proposal

The current directory and single file/blob pages do not have a proper h1/h2 to act as a title of the page.

• Use the current directory name or the file name as the title of the page.
• Include the file type icon to the title as a visual reference.
• If a directory, use the folder-open icon.
