Simplify Breadcrumbs

Moodle provides very limited options for controlling breadcrumbs. With the dashboard set as the default home / landing page this leaves us with three links all taking users back to the same place:

1. The site logo links to the homepage which redirects back to the dashboard
2. The dashboard link
3. The home link in breadcrumbs redirects to the dashboard

Additionally the "courses" link is of no use to students and very little user to academics, it is only relevant to administrators.

We would benefit from adding controls for the breadcrumbs into the theme which allowed us to disable / remove:

1. The "home" link
2. The "courses" link
