Update community relations team's Slack channels in the handbook

Why is this change being made?

While reviewing the slide deck for Marketing Functional Conversations in this Slack request (internal), I figured that the specific team channels for Developer Evangelism, Contributor Program, Education Evangelism are not documented in the community relations landing page.

This MR intentionally duplicates Slack channel information that is otherwise hard to find in the different team handbooks linked below, for better collaboration.
