Handbook update - GitLab Forum Deprovisioning steps

Why is this change being made?

• Fixes minor typos in the deprovisioning steps in the Forum Workflow
• Updates deregister to deactivate to match Discourse UI
• Adds a step for setting the users' profile picture to the default
• Adds a step for removing custom title (sometimes it also contains staff)
