Update CM Scorecard screenshots to better reflect how to score failed tasks

Why is this change being made?

During a recent CM Scorecard study, we confirmed how to score ratings for failed tasks (see Slack screenshot below), and realized that the screenshot example did not reflect the failed task steps as closely as it could. This change updates the screenshot to better represent scoring in the spreadsheet, and provides an additional note to clarify how to score.
