Docs: Update locations of moved Monitor files

Why is this change being made?

The Monitor docs were reorganized last week. This MR updates some prominent pages that linked to the old Prometheus page, which was split apart. I consider this work part of the reorganization and I don't want to force the PMs to go hunting for doc links they need to change.

Related to gitlab-org/gitlab#227441 (closed) where I'm tracking the full scope of the work.
