Advise triagers to be careful with severity labels
Why is this change being made?
Under-estimating severity in triage can have extremely negative consequences. This adds a note in the triage guide to advise triagers to seek domain expert support when unsure of which severity label to apply to a bug issue.
This MR was motivated by a discussion in an issue.
