Clarify error column in Dovetail and issue CMS templates

Why is this change being made?

Both the CMS templates in Dovetail and in CMS issues contain a column for tracking errors, but errors aren't mentioned in the CMS Handbook page. I wasn't sure how to define an error, and whether or not the error rate has any affect on the CMS scores or success/ failure rate. I'm adding this section to help add clarification on that.
