Establish default issue template for reporting problems

While creating the initial draft of the blog post for the NGINX MR in charts, it was noted that a new issue in the GitLab Operator project needed a template to guide submitters on what information would be useful.

This template is a copy of the GitLab Chart to use as a baseline and modified accordingly.
