Adds revert template for security MRs

What does this MR do and why?

On the previous security release, two security fixes had to be reverted because they impacted GitLab.com availability or introduced bug to the codebase. Links:

The approach for each of them depended on some variables. This MR template aims to describe the different options to be chosen based on those variables, and expects to serve as guidance to stage teams when they need to revert a security merge request.
