Doc: Make schema repair instructions for 16.2 more precise

What does this MR do and why?

This doc clarifies the instructions to fix the schema during the 16.2 upgrade for self-installed instances, since when the rails environment is not specified the rake job will be attempted for all of them and some may not exist, leading the rake job to fail.
