Remove LOAD_BALANCER_PARALLEL_DISCONNECT env variable

What does this MR do and why?

The LOAD_BALANCER_PARALLEL_DISCONNECT ENV variable was enabled (rollout issue) and the new code used during the PG14 upgrade. We can now remove this "feature flag" to simplify code.
