SM Duo Pro trials: Include screenshot and additional form info

What does this MR do and why?

We are still seeing issues with customers struggling to fill out the SM trial form. This MR will add additional details on how to properly fill this form out, including:

  1. Adding a screenshot of where to find Subscription Name
  2. Clarifying that email needs to match that of the Subscription Contact

This is a follow-up to the documentation added in !149002 (merged).
