Add details for fix available and false positives in MR approval policy docs

What does this MR do and why?

  • Based on discussion in Slack, clarify the behavior of Fix available in policies
  • Clarify how to use False positive attribute while we're at it
  • Fix instance of SaaS used as guidance states to use GitLab.com
  • Remove known issues section as most items were addressed
  • Moved one item for "Newly detected CVEs" behavior into it's own troubleshooting section
