Reuse hosts during service discovery update

What does this MR do and why?

Every time a new host gets added to the host list during service discovery, all the current host connection pools get discarded and then recreated unnecessarily to connect to the exact same databases again. The purpose of this MR is to prevent the existing hosts from being replaced.
