Update Praefect Documentation

What does this MR do?

Updates the Praefect documentation. The current arch diagram is confusing and would only be used in the case of someone migrating to multiple Gitaly nodes behind Praefect. This MR removes the original Gitaly node from the documentation and enforces consistency of the node names in the architecture and subsequent paragraph.
