Update server hooks documentation to current behavior

What does this MR do?

Updates the server (custom) hooks documentation to match current behavior. The project-specific hooks directory no longer works as that's been deprecated/dropped during the move to Gitaly. Since this was previously a symlink this wouldn't work for custom hooks anyway.
