Improve Gitaly HA docs

What does this MR do?

Improves the Gitaly HA docs based on the demo last week.

• Adds better diagram
• Clarifies exactly how many nodes needed
• Warns that SQL needs to be a separate server
• Make it more explicit that Gitaly needs to be configured 3 times
• Simplifies Gitaly config so that it's the same for every node
• Clearer victory step.
• Add migration docs
