Remove Recommendation to Use add_column for Small Tables

What does this MR do?

Remove the recommendation to use add_column along with change_column_default for small tables from https://docs.gitlab.com/ee/development/migration_style_guide.html#adding-columns-with-default-values.

Suggesting to use add_column for "small" tables has caused confusion - when is it ok, when should you use the other helper.
