Add a design suggestion template for creating a new issue

What does this MR do?

Adds a new template for submitting issues for a design suggestion. Before I started working at GitLab, I had design related ideas but wasn't sure how to format information so that it would be seen. The intent of this template is to encourage everyone in the community that they can contribute to the design direction of GitLab.
