Add Atlassian referrer to Jira links inside GitLab

What does this MR do?

This adds a referrer property defined by Atlassian to all Jira links inside GitLab, on gitlab.com and self-managed instances.

Introduces a method Integrations::Jira#web_url to unify the URL generation.

Implements #323487 (closed), and should also fix #329420 (closed).
