Revise tiers on rules page

What does this MR do?

• Merge Request Approvals are available in all tiers.
• Merge Request Approval RULES are a paid feature.

Let's hammer out some language to clarify this in https://docs.gitlab.com/ee/user/project/merge_requests/approvals/rules.html (and possibly other pages) to make the differences clear.
