Mention Monitor group instead of APM group

What does this MR do?

While investigating #321341 (comment 663935610) I've stumbled upon some broken links which led to "APM group".

This MR changes all 4 occurrences of "APM group" to "Monitor group".

This MR also renames all occurrences of "Health group" to "Monitor group" to mirror category/group structure more correctly.
