Fix version history in commit_templates.md, minor example update

What does this MR do and why?

This is a minor doc fix after !75639 (merged) was merged:

  • It added line Added url, approved_by, and merged_by variables in GitLab 14.6. to version history, but the MR milestone was changed to 14.7 before the merge, so I'm assuming this won't be available in 14.6.
  • Mails in the example were changed from @gitlab.com to @example.com during review, but one was left unchanged as user@gitlab.com - I've updated it.
  • The examples and description didn't make it clear that field Full name from user profile is used, so I've proposed changing User A, User B and Some User to Sidney Jones, Zhang Wei and Alex Garcia.
