Remove support for Terraform 0.12

Matt Kasa requested to merge remove-terraform-0.12 into master

We are currently maintaining support for the latest 4 versions of Terraform and v1.0.0 has just been released, which would increase the number of versions we support to 5, this removes the oldest version in preparation for adding 1.0.
