- Updated things for Jekyll 3
- Added some gems to replace certain files including
  * jekyll-sitemap - replaces the sitemap.xml file
- Removed the need for grunt by making use of the new(ish) sass support in
  Jekyll
- Removed the pages plugin and moved pages to their own folder
- Changed feed.xml layout to null and removed xsl & css styling elements
- Removed unnecessary Rakefile
- Updated README and LICENSE with current dates and change of hosting info
- Changed thumbnail images for posts, pages, categories and trips to full links
  instead of flickimg tags
